To what extent do perceptions, definitions and attitudes towards?

I'm sure that perceptions, definitions and attitudes differ in every part of the world, for many reasons. For instance, some people are just morons and act educated on matters when they aren't. Many have been extremely hurt by several with cluster B disorders and lump everyone all together. That in itself can be black and white thinking. However, the behaviors of most cluster B individuals are eerily similar. Many don't lump bpd/npd together as you may think. For instance, I was literally married to someone with bpd, but she has heavy npd traits also. As we all know, bpd and npd can mimic each other almost exactly. However, the intent seems to be different. The problem is, that there are many co morbidities that go along with either diagnosis. The co morbidities can determine how a person with either disorder can act. For instance someone diagnosed with bpd and bi polar together can create a whole new set of issues, than someone with a diagnosis of straight bpd. Everybody knows that there are millions and millions in the US alone that walk around undiagnosed and untreated everyday. Ignorance exists in every corner of the globe, however there is a tremendous of Ignorance here in the United States. I think that it boils down to a lack of being educated on these disorders and plain old Ignorance.
